This form is used by persons coming in who have access to a printer.  This packet could be printed and filled out, focusing on identifying the debts and the approximate balances.  Doing this before coming in might be convenient for you.  When scheduling an appointment if you mention that you have done this, it would be helpful.

You might want to use this as a supplement to the initial information sheet, if you have more budget concerns.
